Notification of a discovery
This form is for section 89 of the Queensland Heritage Act 1992 and giving notice about a discovery of historical archaeology. More information about this is available at Report a discovery.
Before you start
Please be aware that you cannot save this form and continue to fill it out later. It may take over 15 minutes to complete, so it is recommended that you prepare the information you will submit beforehand. Alternatively you can use Notification of a discovery and email your application.
Please submit as much detail as possible by completing all fields and providing additional attachments. To complete this form you will need:
- location details (address and/or co-ordinates)
- assessment of the discovery (of local-level or state-level cultural heritage significance)
- management activities already undertaken
- proposed management activities
- map and site plan
- photographs.
If this is regarding a discovery of Aboriginal or Torres Strait Islander heritage, please do not use this form but contact the Cultural Heritage Unit in the Department of Seniors, Disability Services and Aboriginal and Torres Strait Islander Partnerships by email to cultural.heritage@dsdsatsip.qld.gov.au.
If this is regarding fossilised or palaeontological remains, please do not use this form but contact the Queensland Museum.

Privacy
The personal information collected on this form will be used by the Department of Environment, Science and Innovation to administer the provisions for artefacts in Part 9 of the Queensland Heritage Act 1992. The personal information will not be disclosed to other parties without your consent (and where relevant, the consent of third parties identified in the notification), unless required or authorised by law.
The personal information may be stored in department databases and will be retained as required by the Public Records Act 2008.
