Use this application form if you or the person you are applying for were born or adopted (and registered) in Queensland.
New parents note: you must register the birth of your child before you can get a birth certificate.

Privacy notice
The Department of Justice is collecting your personal information for the purpose of processing your application for birth certificates under the Births, Deaths and Marriages Registration Act 2023 and Surrogacy Act 2010. The information on this form may be provided to law enforcement agencies and to government and non-government agencies for verification of the data. Access to this information or a certificate may be granted to any person who has adequate reason to obtain it, or who meets the requirements of the access policy.
Purchasing a birth certificate on a smart device may be convenient.

If you can't complete the order online, you can use the PDF application form and apply by post or in person.
Submit your application and payment for a birth certificate to us:
- by post to Registry of Births, Deaths and Marriages, PO Box 15188, CITY EAST QLD 4002
- at one of our agents at a Queensland Magistrates Court or Queensland Government Agent Program (QGAP) office (except the Brisbane Magistrates Court).
