Fill in to alter the sex on your birth certificate
Use this application form to change your record of sex if you are 16 or older and were born or adopted in Queensland.
Then you will need to print the application form to complete the statutory declaration and supporting statement before submitting it along with supporting documents.
Step 1—Complete this part online
- Start by answering the question below—it should take you less than 15 minutes.
- Choose how you would like to pay.
- Review your details.
- Read the email we send you to find out how to finalise your application.
- Download, print and sign the the filled-in application form.
Step 2—Complete on paper and submit
- Visit a Justice of the Peace or another authorised person to complete the statutory declaration section and to certify your proof of ID and supporting documents.
- Have an adult (18 or older) who has known you for 12 months complete the supporting statement section.
- Gather your proof of ID and supporting documents.
Proof of ID documents must be submitted with your signed application. When submitting by post, have your documents certified by a Justice of the Peace or another authorised person.

- by post to Registry of Births, Deaths and Marriages, PO Box 15188, CITY EAST QLD 4002
- in person at the Brisbane registry customer service centre at Level 32, 10 Ann Street, Brisbane.
Step 3—What next
- If your application is approved, you will need to update your details with every organisation you have a personal account with. We suggest that you will need to show them an official linking birth certificate.
