Fill in a notice of intention to enter a civil partnership

Use this application form to register your relationship as a civil partnership with a declaration ceremony. You need to be 18 or older and at least 1 partner must live in Queensland.

Step 2—Complete this part by post or in person

  1. At least 10 full days before your declaration ceremony, and no earlier than 12 months before, you need to
    1. show your proof of ID and eligibility documents to your civil partnership notary
    2. have your civil partnership notary witness your signatures in the declaration section and complete their declaration.
  1. If you are not having your declaration ceremony at the Brisbane registry, a copy of the completed application form must also be supplied to us either:
    1. by post to Registry of Births, Deaths and Marriages, PO Box 15188, CITY EAST QLD 4002
    2. in person at either the Brisbane registry customer service centre at Level 32, 180 Ann Street, Brisbane, or your nearest Queensland Magistrates Court or Queensland Government Agent Program (QGAP) office.

Don't use this form

You can choose to register your civil partnership without a ceremony or get married.

You are not eligible

You can't take part in a civil partnership declaration ceremony in Queensland unless you or your partner lives in the state. You may choose to be married instead.

You are not eligible

Your notice of intention can't be accepted if you or your partner is already married, or in a civil partnership.

You need to prove your identity

A civil partnership notary can't conduct your civil partnership declaration ceremony until you can prove your identity. You can contact us if you need more information about this.
