The Department of Transport and Main Roads (‘the department’) is collecting your personal information on the form for the purpose of assessment, investigation, action and response of your complaint.
Your personal information will be disclosed to the relevant agency/s, integrity body or statutory body to investigate and respond to your complaint. They may, to assist with managing your complaint, pass on the information to other relevant agencies, service providers, local governments and non-governmental organisations that may be able to assist you.
Please note that while every effort is made to ensure your complaint is directed to the correct agency, in some circumstances the receiving agency may need to send your complaint to another agency. This can occur for example, where a complaint is complex and concerns a number of different agencies, or where a machinery of government change has recently occurred.

The complaint form is hosted by Microsoft Azure. The department is not responsible for the privacy, security practices or content of the Microsoft Azure website. To ensure optimal performance and user experience of the department’s website, external service providers may be engaged from time to time. Where this occurs, the department will take all reasonable steps to ensure that your personal information remains protected in accordance with the Information Privacy Act 2009 (Qld).
