Deadly Choices birth certificate initiative

We are proud to be partnering with the Institute for Urban Indigenous Health (IUIH) and Townsville Aboriginal and Islander Health Services (TAIHS) to promote the popular Deadly Choices commemorative birth certificates and remind new parents to register their child.

Starting in 2016, the initiative helps Aboriginal and Torres Strait Islander children get ahead in life. It helps ensure their births are registered and that they get a birth certificate so they can access education and health services.

Who can apply

Parents of children (5 or younger) can apply for a Deadly Choices birth certificate package (including a standard birth certificate), if they are:

  • patients at a participating IUIH or TAIHS clinic
  • up to date with their vaccinations and health checks.

How to apply

To apply for your child's free birth certificate package, you need to:

  1. Pick up a Deadly Choices birth certificate package application from reception at your clinic.
  2. Complete the application form.
  3. Submit your application form in person, by email or by post.

This application form is only available from participating clinics and is not available from us.

Return the completed application form to the staff at your clinic.

Scan the completed application form and email it to BDMCommunityLiaison@justice.qld.gov.au.

We visit Aboriginal and Torres Strait Islander communities to help with registering bubs’ births and to help parents get birth certificates.

Come and have a yarn with us to get some help with applications.

If your child is between 0 and 16 years old and is yet to be registered, come along and have a yarn with our Community Engagement Officer, David. He will help you register your child's birth and get a certificate.

All new birth registrations will receive a free birth certificate.

You will need to bring ID. This may include:

  • driver licence
  • Medicare card
  • bank card
  • government letter
  • Centrelink health benefit card.

But if you don't have ID, no worries—David will work it out with you.
