Use this application form to end a civil partnership registered in Queensland.

If your civil partnership was registered outside of Queensland, contact us.

You can't use this form to end a marriage—see the Federal Circuit and Family Court of Australia for information on ending a marriage.

Step 1—Complete this part online

  2. Download and print the completed application form.

Step 2—Complete on paper and submit

  1. Visit a Justice of the Peace (with your partner if both applying) to certify your proof of ID and completed application form.
  2. Submit your application form and supporting documents with payment to us either:
    1. by post to Registry of Births, Deaths and Marriages, PO Box 15188, CITY EAST QLD, 4002.
    2. in person at the Brisbane registry customer service centre at Level 32, 180 Ann Street, Brisbane.

        You're almost done

        You will need to submit your application along with payment, proof of ID and supporting documents (as required). When only one partner signs the application, you must also complete and submit the statutory declaration (single applicant).

        If the original official certificate of registration in Queensland is returned, we will replace it with a new one that shows the end of civil partnership note on the back.
