Apply for financial assistance for victims of crime—part A
You can use this form to apply for financial assistance from Victim Assist Queensland if you are a primary or related victim of violence committed in Queensland.
There are different application forms if you are a:
- parent secondary victim—a parent who was injured by learning about acts of violence that were committed against their child (who was under 18)
- witness—a person who was injured by seeing or hearing acts of violence being committed against someone else
- person who has paid for or will be paying for the funeral of a homicide victim or for the funeral of an unborn child who was lost because of violence (but is not a related victim).

Violence that happened before 2009
You may only get financial assistance in some cases if the violence happened before 1 December 2009. Learn more about the different rules that apply.
Application support
We acknowledge that it may be hard for you to apply for financial assistance because you need to tell us about what has happened to you. You may want to fill in this form privately or ask a friend or victim services worker for support.

Before you start
You need to report the crime before you apply. Please follow our reporting requirements.
If you reported it to police, you’ll need your reference number (QP number, if known) or other information about who you spoke to.
At the end of the form, we’ll ask you to upload supporting documents—you need to photograph or scan:
- an approved ID card or document from our list.
- approved documents about your injuries (if you have any).
- a special primary victim report (if relevant).
If you scan your documents, we recommend scanning each one separately.
You’ll need your banking details (BSB and account number) or a safe postal address (if you prefer a cheque).

In most cases, the person making this application needs to be over 18.

You will also need to tell us if you have received or are going to claim money from other sources because of the violence. The form will ask you about this.

Other documents you may need
You’ll need to upload a copy of a document that confirms you’re the child’s parent or guardian (e.g. the child’s birth certificate, family Medicare card or court order) if the application is for a child.
You’ll need to upload a copy of a document that confirms you’re the adult’s guardian, have enduring power of attorney or are their administrator if the application is for an adult with impaired capacity.
You’ll need to upload a copy of a document that confirms you have authority to act on behalf of the victim if you are their lawyer.
You’ll need to provide the date you were given permission by Victim Assist Queensland to apply on behalf of the applicant/victim.
