Changed your name?

If you have changed your name, you need to provide us with supporting documentation, such as:

  • a photocopy or scan of a marriage certificate
  • your Queensland driver licence number, Centrelink customer reference number (CRN), or Commonwealth Department of Veterans' Affairs (DVA) file number

and email us: cardapplication@smartservice.qld.gov.au
