How to find information on your personal documents
When you are securing your Queensland Digital Identity, you will be asked to provide your information from your personal documents.
Here are the documents you may be asked for and where to find your information on them.
- Queensland marine licence

- Visa (using your foreign passport)

Australian passport
If you have only one name on your passport, enter the name in the Family name field and leave the Given name/s field blank.
Your passport cannot be verified online if it:
- has been cancelled (replaced, reported lost or stolen)
- has been expired for more than 2 years.
If you have renewed your passport, you need to verify the new passport.
Medicare card
Your Medicare card cannot be verified online if it has been reported lost or stolen.
Medicare card – green

Visa (using your foreign passport)
Enter the details exactly as they appear on your foreign passport you used to get your Visa.
Your Australian visa cannot be verified online if:
- it was issued before 1 July 1990
- it’s expired, cancelled or has been re-issued
- it doesn’t display your full date of birth (day, month and year)
- you recently received Australian citizenship
- you’re a temporary resident not currently in Australia.
Australian certificate of registration by descent
If you're asked to enter a document number:
- for certificates issued after 2007, you can enter the client ID number
- for certificates issued before 2007 you can enter the register number and entry number, separated by a slash '/' with no extra spaces. For example, if the register number is 243 and entry number is 87, enter 243/87.
Australian citizenship certificate
Ensure that you:
- enter your name exactly as it appears on your certificate
- enter your date of birth in the order day/month/year (DD/MM/YYYY). If it does not have your full date of birth, it may not verify online
- enter the correct Document ID:
- Stock Number (found on back) is typically 1-3 alpha characters, a number in brackets, followed by numeric characters. Enter into Stock Number field removing any special characters and spaces. Example: ACC (1) 23456 should be entered as ACC123456
- the Evidence Number is 11 numbers (found on front).

Immigration card (ImmiCard)
Ensure that you:
- enter your name exactly as it appears on your ImmiCard
- enter your date of birth in the order day/month/year (DD/MM/YYYY) – if it does not have your full date of birth, it may not verify online
- do not use any spaces when entering your ImmiCard number – you should only use the 3 letters and 6 numbers, for example ABC123456.

Certificate numbers will be printed on certificates issued after 1/05/2002. Certificate numbers provide a more reliable check and, where available, should be used in favour of the registration number.

For some certificates the registration number is in the top right corner.
Northern Territory
Registration number
Northern Territory registration numbers have three different formats, depending on when a certificate was issued:
- registrations numbers prior to 1999, Only enter the numbers after the slash
- registrations after 1999 will have a straight 8-digit registration number
- only numbers after the last punctuation character should be entered, excluding any alpha characters.
Certificate Numbers
- Certificate numbers will be printed on certificates issued after 12/07/1999.

For some certificates the registration number is in the top right corner.
New South Wales
Registration number
Exclude any generic characters, such as a slash or the year. For example, if your registration number is formatted as 1974 / 1234, enter only 1234.
If your birth certificate was issued:
- before 1952 and does not have a registration number, it cannot be verified online
- between 1952 and 1974 and doesn’t have a registration number, enter only the district number in the Registration number field. (If your certificate was issued in 1970 or 1971, the district number may not verify.
Certificate numbers
Will be printed on certificates issued after 13/10/2014. Certificate numbers provide a more reliable check and, where available, should be used in favour of the registration number.

Re–registered certificates
If your certificate was re-registered due to an update, correction, name change or legitimation before or during 1993, it may not verify. This is because the certificate registration number and year were changed.

Will be printed on certificates issued after 18/02/2019.

Western Australia
Registration number
The registration number for Western Australia birth certificates is formatted in different ways depending on the year it was issued.
You need to enter 7 digits in the Registration number field:
- if your registration number starts with 7 digits, enter the 7 digits and do not include any letters
- if your registration number has fewer than 7 digits, add zeros at the front
- if your certificate was issued before 26 November 2001, it will have a district number before the registration number and district name. Enter both the district number (as 2 digits) and the registration number (as 5 digits) in the Registration number field. Add zeros at the front of each number as necessary.
Certificate Numbers
- certificate numbers will appear on certificates printed on or after 29/03/2017. Where possible, to provide a more reliable check, the certificate number should be used instead of the registration number
- the certificate number should always be 11 digits in length. If the number is less than 11 digits, add leading zeros to equal the required amount.
Marriage certificate
Your official marriage certificate needs to be issued in Australia.

Registration numbers on Northern Territory marriage certificates can be made up of numbers and letters.
Enter only the numbers after the last letter or punctuation character in the Registration number field. For example, for the registration number:
- NT 123/45, enter 45
- 136 book 40, enter 40.

Registration number
You’ll need to enter both the district and registration number in the Registration number field.
The registration number should be 7 digits long. To achieve this:
- for district numbers 2-9, add a zero in front of the district number
- add additional zeros in front of the registration number to make it 7 digits long.
