Mental health experience surveys
Your feedback helps us improve
You can give feedback about your experience with Queensland mental health services at any time.
We also use surveys to collect feedback from people who have used our mental health services, including carers and family members.
This helps us understand how services are performing and how they can improve.
All hospital and health services across Queensland take part in the following surveys.
- Your experience of service (YES) survey
- Carer experience survey (CES)
- Family of youth (FoY) survey
Your experience of service survey
This survey asks about your experience using mental health services.
It runs for 6 weeks. If you use a mental health service during that time, staff will give you a paper survey or a link to the online survey. Paper copies are available in 24 languages.
The YES survey is a national measure developed with consumers. We started using it in Queensland in 2015. It’s based on the recovery principles in the national standards for mental health services.

Carer experience survey
This survey asks carers about their experience with mental health services.
It runs all year and you can fill it in online or on paper. We'll send you a paper version to fill in, or email you a link to the online survey.
The CES is a national measure developed with carers. We’ve used it in Queensland since 2019.
Family of youth survey
This survey asks parents, carers and families of young people about their experience with mental health services.
It runs for 6 weeks. If a young person you support uses a mental health service during that time, staff will give you a paper survey or a link to the online survey.
Your privacy
We respect the privacy of people using mental health services, and we protect your information.
Taking part in a survey is voluntary. You can skip any question, including demographic questions.
When you fill in a survey, we link it to the service using a code. This code doesn't identify you. We only share results with services once they have at least 10 completed surveys. This protects your privacy and keeps individual feedback anonymous.
We store responses securely and analyse the results.
