Legal responsibility

You have a legal responsibility to tell us when you have a permanent or long-term medical condition that’s likely to affect your ability to drive safely. You also need to advise us if there is a permanent or long-term increase of a medical condition you’ve already told us about.

You may need a medical certificate for drivers

When you report your medical condition, you’ll need to give us a medical certificate unless you’re only reporting that you need to wear glasses or contact lenses to drive.

How to report your medical condition

Report online

Use our online service to notify us of your medical condition. You can do this with or without a medical certificate.

Report by email, post or in person

  1. Complete a medical condition notification (F4355)
  2. Include your Medical Certificate for Drivers with the form, if possible
  3. Email to mcr@tmr.qld.gov.au
