If you wish to apply for information about the identity of an adopted person or birth parent/s please complete the relevant application form below and return it to Adoption Services.
As part of your application, you must provide a certified copy of two documents from the following list to prove your identity:

If you are a birth parent and you wish to apply for identifying information about your child who is under the age of 18 years:
If you are a relative of a birth parent, where the adopted person is now an adult, and wish to apply for adoption information on their behalf (a relative may apply only where the birth parent is deceased and a relative or guardian may apply where the birth parent does not have capacity to apply for themselves):
If you are a relative of a birth parent, where the adopted person is still a child, and wish to apply for adoption information on their behalf (a relative may apply only where the birth parent is deceased and a relative or guardian may apply where the birth parent does not have capacity to apply for themselves):
If you are the relative of an adult adopted person and wish to apply for adoption information on their behalf (a relative may apply only where the adult adopted person is deceased and a relative or guardian may apply where the adult adopted person does not have capacity to apply for themselves):
A pre-adoption sibling is a person who would have been an adopted person’s brother or sister if the adoption had not taken place and is not an adopted person themselves.
Information can be given only if the adopted person provides written consent and where a birth parent has advised Adoption Services (in writing) that they are agreeable to be contacted.
If you are an adopted child (under the age of 18 years) and have the consent of your parent/s to apply for pre-adoption information:
Adopted people, adoptive parent/s, birth parent/s, relative/s and birth sibling/s (a brother or sister who was not adopted) can request information about past adoptions in Queensland.
If you wish to search for a birth relative, we encourage you to make contact with an adoption counselling and support service to help you prepare for some of the challenges associated with searching for, and possibly meeting, your birth relatives.
