Evidence of identity
Important information
- All documents must be originals—photocopies and certified copies are not acceptable.
We need evidence of your identity before we can give you a Queensland driver licence or replace an existing licence. This evidence may also be needed for other transactions, including registering a vehicle.
About evidence of identity
You can give us this evidence by showing your Queensland-issued:
- current driver licence (or 1 that has been current at any time within the last 2 years)
- adult proof of age or photo identification card
- industry authority card for example a taxi driver licence
or - marine licence indicator.
If you do not have these, you will need to present 3 original evidence of identity documents:
- 1 category A document + 2 category B documents
or - 2 category A documents + 1 category B document.
You will have to show more documents if your:
- name has changed
- Queensland residential address is not shown on your documents.
All documents must be originals—photocopies and certified copies are not acceptable. The department may keep a copy of your evidence of identity documents or information about them to verify these documents with the issuing agency or a document verification service.
Category A documents
Category A documents show evidence of the legal existence of your name and date of birth.
All Category A documents must show your full legal name.

Category B documents
Category B documents show evidence of the use of your name in the community. We will accept a Category B document that shows your initial/s and surname if it matches the full name on your Category A document.

*Digital versions of these documents can be presented for in-person transactions only. Customers must, when presenting a digital copy, login to the relevant app and show the card within their digital wallet. Screen shots or printed copies are not accepted.
Evidence of your name change
If you have changed your name, or the details of your name are different on your identification documents, you must show an original, official document that provides evidence of your change of name.

Overseas marriage certificate requirements
We will accept an official overseas marriage certificate if you also provide:
- 1 category A document in your married name
or
- 2 category B documents in your married name.
The overseas marriage certificate must show an official crest and the registration number. If the marriage certificate is not in English, a recognised English translation from a translator approved by the National Accreditation Authority for Translators and Interpreters (NAATI) is required.
Evidence of your Queensland residential address
If your evidence of identity documents do not show your current home address in Queensland you will need to prove that you live in Queensland.

If you genuinely cannot show any of these documents to prove your address you can:
- provide a statement from your employer. The statement must be on your employer’s letterhead, include the business registration number (ABN, ACN) as well as your full name and residential address. It must also contain the full name, daytime contact number and signature of a person authorised to sign on behalf of your employer. While you may be authorised to sign on behalf of your employer, you cannot complete your own statement.
- provide a completed and verified Customer Identity Declaration - First Nations Person form F4168
- provide a statement from Defence Housing Australia (if you are an Australia Defence Force member or their immediate family).
If you are under 18 and cannot show any of these documents to prove your address, you can:
- visit a transport and motoring service centre in your school uniform
OR - present your student identification card
OR - have a parent or guardian attend with you and show their driver licence with the same address.
Australian Defence Force members and their immediate family
If you are in Queensland on full-time permanent service with the Australian Defence Force (ADF) or the Defence Force Reserve and can’t provide evidence of your Queensland residential address from the acceptable documents list, you can provide a statement or confirmation from Defence Housing Australia. This can be a paper or electronic statement on Defence Housing Australia letterhead that includes your:
- full name
- Queensland residential address
- entitlement to accommodation, which is paid out of the defence force member’s salary.
You must also show a current Australian Defence Force photo identification card.
Immediate family members with a current civilian ADF identity card can also use this option. An immediate family member is someone who:
- lives with an ADF member
AND - and is entitled to be transferred with the defence force member, at the expense of the ADF.
Deployed Defence Force members
If you are deployed and need someone to attend a transport and motoring service centre on your behalf, both you and your authorised representative must complete the Authority to represent Registered Operator Vehicle Registration Regulation (F5225) form.
Your authorised representative will need to present a Category A OR Category B document as evidence of identity for themselves.
They will need to provide any of the following as evidence of identity for you (original documents only):
- a Queensland driver licence, current or expired less than 2 years
- Qld Digital Licence PDF valid at any time within the last 2 years
- 1 Category A document + 2 Category B documents or
- 2 Category A documents + 1 Category B document.

Evidence of identity for customers under 18
If you’re under 18 and don’t have identification that was issued by us (like a Queensland driver licence, marine licence or photo identification card), you may have access to fewer documents to use for evidence of identity. We've gathered examples of evidence that under 18s are most likely to have—refer to the evidence of identity and evidence of residency for the full lists.
Any documents you use for evidence of identity must be original – we can’t accept copies, scanned documents or certified copies.
You must present:
- 1 Category A document and 2 Category B documents
OR - 2 Category A documents and 1 Category B document.
If your documents don’t include your Queensland residential address, you must also show proof that you live in Queensland.
If you have changed your name, or the details of your name are different on your identification documents (for example, if you have a different last name on some documents due to divorce), you must show evidence of your name change.
Category A documents for customers under 18
Category A documents show evidence of your legal name and date of birth.
Birth certificates
We will accept an Australian birth certificate as evidence of your legal name and date of birth.
You must provide a full birth certificate (not an extract or a commemorative birth certificate).
You can apply for a replacement Queensland birth certificate if you don’t have one, or apply for one from your home state if you weren't born in Queensland.
Passports
If you don’t have an Australian birth certificate, we will accept a passport from Australia or another country.
The passport must be current or expired by less than 2 years.
Immigration documents
We will accept a current Australian citizenship or naturalisation certificate.
We will accept any of the following documents issued by the Department of Immigration and Border Protection or Department of Home Affairs, for up to 5 years after the date of issue:
- Australian Migration Status (AMS) ImmiCard
- Permanent Resident Evidence ImmiCard
- Evidence of Immigration ImmiCard

Category B documents for customers under 18
Category B documents show evidence of your name, in use within the community.
If your document has your initials and last name instead of your full name, we will accept it if it matches the full name shown on your Category A document.
Medicare card
We will accept a current Medicare card that you are listed on.
If you have a digital version of your Medicare card, you can present it by logging into the relevant app and showing the card to us. You can’t use a screenshot or print-out from the app.
You need to present your physical or digital Medicare card to us (in person), even if you provided your Medicare card details to enrol in PrepL.
Student identity document
We will accept a current student identity document (such as a student ID card) if it includes a photo.
If you have a digital version of a student identity document, you can present it by logging into the relevant app and showing us the document. You can’t use a screenshot or print-out from the app.
We will not accept school certificates or statements of enrolment.
Bank card
Some banks allow customers who are under 18 to open an account.
We will accept a current debit, EFTPOS or credit card if your name is embossed or printed on the card.
If your name isn’t on the card or you have a passbook, you will need a supporting letter from your bank that confirms your name and the address linked to that account.
Services Australia BasicsCard
We will accept a current Services Australia BasicsCard as a Category B document.

Evidence of Queensland residential address for customers under 18
If your evidence of identity documents don't show your current home address in Queensland, you will need to prove that you live in Queensland.
We will accept any of the following documents:
- an Australian Tax Office (ATO) assessment/tax file number confirmation notice from the current or previous financial year (block out your tax file number before submitting this document)
- an official letter from Centrelink or the Department of Human Services issued in the last 6 months
- a statement from your employer
- a completed and verified Customer Identity Declaration - First Nations Person form F4168
If you don’t have any of these proof of address documents, you can:
- visit a transport and motoring service centre in your school uniform
OR - present your student identification card
OR - have a parent or guardian attend with you and show their Queensland driver licence with the same address.

Evidence of name change for customers under 18
If you have officially changed your name, or your name is not the same on all of your identification documents, you must show evidence of your name change.
Items you may be able to use include:
- an Australian Change of Name certificate issued by the relevant Registrar of Births, Deaths and Marriages
- an Australian birth certificate (amended with/without notations) issued by the relevant Registrar of Births, Deaths and Marriages
