Proof of identity
When applying for a financial grant following a natural disaster through the Community Recovery Portal, you must provide two (2) separate forms of identification (ID). One (1) must have a photo, and one (1) can be another non-photo form of ID. If no photo ID is available, you can instead provide three (3) forms of non-photo ID. See the list of accepted ID categories below.
If you are unable to provide these, we accept other forms of ID. Please note that you will need to call the Community Recovery Hotline on 1800 173 349 to provide other forms of identification.
The two (2) best forms of ID to apply are:
- Driver licence (Australian and international)
- Medicare Card

Applying for dependants
If you are applying on behalf of your dependants, one of your forms of ID must include the following:
- Queensland driver licence
- Medicare card
- Health care card
- Private health card
- Pension Card.
