Apply for an individual disability parking permit

You can apply for a disability parking permit for your own use. You can also apply on behalf of an applicant as an agent/ parent / carer.

There are 2 types of disability parking permits:

  • Australian disability parking permit
  • Red permit—for existing red permit holders only.

If you have an Australian disability parking permit issued interstate and you move to Queensland, you will need to apply for a new permit within 14 days of becoming a Queensland resident.

Eligibility

You can apply for an Australian disability parking permit if you are a Queensland resident and one of the following applies to you:

Mobility impairment

  • Unable to walk and always require the use of a wheelchair.
  • Ability to walk is severely restricted by a permanent medical condition, or ability to walk is severely restricted by a temporary medical condition, or disability that you will have for 6 months or more as certified by your health professional.

Vision impairment

  • You have been diagnosed as 'legally blind'. This includes temporary (for 6 months or more), or permanent basis.

An intellectual, psychiatric, cognitive or sensory impairment alone does not meet the eligibility criteria, unless you have an eligible mobility or vision impairment.

Permit cost

You will need to pay $19.75 for a:

  • new Australian disability parking permit application, where you are not an existing Queensland permit holder
  • 6 to 12 month Australian disability parking permit application
  • 5 year Australian disability parking permit application—when your permit has been expired for more than 3 months.

An application cannot be processed without payment of the non-refundable fee.

Applicants with a mobility impairment

Apply online

You may be required to provide medical certification with your online application. Your doctor or occupational therapist will need to complete 'Part 2 - Medical details' of the Australian Disability Parking Permit—Mobility Impairment Application (Individual) (F4814) and you will need to scan and attach it when applying through this online service if:

  • you do not already have a permit
  • your permit expired more than 3 months ago
  • you have a current or expired temporary permit (6 to 12 months).

Apply in person or by post

  1. Download the Australian Disability parking permit—Mobility Impairment application (individual) (F4814) and complete 'Part 1 – Applicant details'.
  2. Have your doctor or occupational therapist complete 'Part 2 - Medical details' and pay the fee if:
    • you do not already have a permit
    • your permit expired more than 3 months ago
    • you have a current or expired temporary permit (6 to 12 months).
  3. Lodge your completed application in person at your nearest transport and motoring customer service centre, or by post, and pay the fee. Do not send cash by post. Make your cheque or money order payable to the 'Department of Transport and Main Roads'. Post your completed application with cheque or money order to:

Applicants diagnosed as legally blind

Apply online

You will need to provide a copy (front and back) of one of the following cards:

  • Vision Impairment Travel Pass—issued by Translink
  • Centrelink Blind Concession Card
  • Department of Veterans' Affairs Blind Concession Card

If you do not have one of those cards, provide medical certification with your application. You will need to scan and attach a completed copy of 'Section 3 – Legally blind assessment' and 'Section 4 - Doctor, Optometrist or Ophthalmologist certification' of the Australian Disability Parking Permit—Legally Blind Application (Individual) (F5280) to apply through this online service.

Apply in person or by post

  1. Download the Australian Disability Parking Permit—Legally Blind Application (Individual) (F5280) and complete:
    • Section 1 of the application
    • Section 2 and attach a copy (front and back) of one of the following cards to the application:
      • Visual Impaired Travel Pass—issued by Translink
      • Centrelink Blind Concession Card
      • Department of Veteran Affairs’ Blind Concession Card

Or

  1. Ask your medical professional to complete Section 3 and Section 4 of the Australian Disability Parking Permit—Legally Blind Application (Individual) (F5280).
  2. Lodge your completed application in person at your nearest transport and motoring customer service centre, or by post, and pay the fee. Do not send cash by post. Make your cheque or money order payable to the 'Department of Transport and Main Roads'. Post your completed application with cheque or money order to:

Application decision

You will receive notification of approval or refusal of a permit within approximately 28 days from when your application is received. Notification will be sent to the address provided on the application form.

Request a decision review

If your application was refused, you can have it reviewed within 28 days.

To have your application reviewed complete an application for review of a decision (F3859) and take it to your nearest transport and motoring customer service centre.
