Information for doctors and health professionals
In Queensland, the Australian Disability Parking Permit eligibility criteria is based on:
- a person’s functional impairment to their ability to walk
or - a person having a visual defect that results in being diagnosed as legally blind.
Eligibility is not based on intellectual, psychiatric, cognitive or sensory impairment alone (except for visual defects that result in legal blindness) unless the person also has a severe impairment to their functional ability to walk or is diagnosed as legally blind.
Applicants that have a cognitive impairment that severely restricts their mobility may be eligible for a disability parking permit. For example, the applicant has suffered a brain injury resulting in a severe restriction to their ability to walk.
Permits are only granted to persons who meet the eligibility criteria. This is to help ensure disability parking bays are available to those who need them most.
Applications made on mobility criteria may be certified by doctors or occupational therapists. Applications made on legal blindness criteria may be certified by doctors, optometrists or ophthalmologists.

How to complete the application form
Certify an application based on mobility impairment
Health professionals certifying an applicant based on mobility impairment need to complete Section 2: Medical Assessment and Section 3: Doctor/Occupational Therapist's Certification of the Australian Disability Parking Permit Mobility Impairment Application (Individual) (F4814).
After an application has been certified it is the responsibility of the applicant to lodge their application form with the Department of Transport and Main Roads.
Certify an application based on legal blindness
The applicant will need to provide a copy (front and back) of their card when they lodge their application:
- Vision Impaired Travel Pass issued by TransLink
- Centrelink Blind Concession Card
- Department of Veterans Affairs Blind Concession Card.
You do not need to certify their application if an applicant holds any of these cards listed.
Health professionals certifying an applicant based on legal blindness will need to complete Section 3: Legally Blind Assessment and Section 4: Doctor, Optometrist or Ophthalmologist Certification of the Australian Disability Parking Permit - Legally Blind Application (Individual) (F5280).
After an application has been certified it is the responsibility of the applicant to lodge their application form with the Department of Transport and Main Roads.
