Medical practitioners

Medical practitioners can create and submit the following forms online:

• cause of death medical certificates (Form 9)
• perinatal supplements (Form 9a)
• medical practitioner report of death to a coroner (Form 1A Section A).

By law, you must complete a certificate within 2 working days of a death.

This system is free and easy to use.

What you can do online

Approved service providers can:

• create, certify and print official cause of death medical certificates
• submit cause of death records to us
• receive status updates
• receive fast notification about any issues we have registering a death record
• view previously submitted cause of death certificates.

How to become an online service provider

You can sign up as an online service provider with:

• Microsoft 365 if you have a Queensland Health email address, or
• a Queensland Digital Identity.
