Medical practitioners
Medical practitioners may create and submit cause of death medical certificates (Form 9), perinatal supplements (Form 9a) and medical practitioner report of death to a coroner (Form 1A Section A) for deaths that occurred in Queensland.
By law, you must complete a certificate within 2 working days of a death.
This system is free and easy to use.
What you can do online
Approved service providers can:
- create, certify and print official cause of death medical certificates
- notify the funeral director so they can accept the body
- submit cause of death records to us
- receive status updates
- receive fast notification about any issues we have registering a death record
- view previously submitted cause of death certificates.

To sign up as an online service provider you will need a Queensland Digital Identity.
